Guidelines and reference paths in the Tuscany Region: the case COPD

In 2008, the Region of Tuscany, ahead of all the other Italian Regions, introduced in the Regional Healthcare Plan for 2008-2010[1]  the medicine of initiative as a model of territorial development of primary care, which adds to the traditional medical approach to the acute illness. The approach based upon the medicine of initiative promotes a multiprofessional model of care for chronic patients (DGR 716 of 3/08/2009) and the Regional Healthcare Plan applies it to five disease areas: diabetes mellitus, heart failure, high blood pressure, stroke and COPD. The slide describes the characteristics of the course of treatment recommended by the regional guidelines for this chronic disease.
